Membership

KAISER CARD holders are obliged to keep their information file accurate by regularly reporting any change in personal details and other pertinent date in writing or through online channels. Kaiser Medical Center may, at its sole discretion and without prior notice, suspend, cancel, revoke, and terminate any card issued and/or privileges for whatever reason.

Data Privacy

By using this HealthCard, the holder allows Kaiser Medical Center to process his/her personal information in order to provide the services indicated herein or as may be requested by the holder of this KAISER CARD.

Use of KAISER CARD

The KAISER CARD remains the sole property of Kaiser Medical Center. This KAISER CARD is not transferable and will be accepted only at Kaiser Medical Center Branches when presented by the cardholder whose name and details are indicated on the card. Cardholders must present the card in conducting any transaction or redemption of promos/exclusive benefits. Kaiser Medical Center reserves the right not to honor the KAISER CARD when the holder has not properly presented the card or is unable to present competent evidence of identity or when, or such other similar circumstances.

The use of Kaiser Card shall not be used in conjunction with other promos of Kaiser Medical Center.

Amendments

Kaiser Medical Center reserves the right to and has the exclusive discretion to amend the terms and conditions and guidelines of Kaiser Card.

Kaiser Medical Center reserves the right to cancel or amend, in any way, the awards, privileges, benefits of Kaiser Card as was previously advertised or offered to the holders.

Kaiser Medical Center reserves the right at all times and without notice, to amend the terms and conditions, including the rules and guidelines of the Kaiser Card. For this purpose, Kaiser Medical Center shall send the revised terms and conditions, guidelines or rules via email after the same has been updated/amended.

Breach of the Terms and Conditions

A member shall be in breach of the terms and conditions of the Kaiser Card when it uses any awards, privileges, facilities, services, or arrangement in a fraudulent manner, with misleading information and misrepresentation. The determination of acts constituting breach is upon Kaiser Medical, whose decision shall be final and conclusive.

Any breach of the terms and conditions, by willful act or omission, may result in the termination of membership, forfeiture of points when applicable, and cancellation of awards and privileges.

THE TERMS AND CONDITIONS HEREIN SET FORTH SHALL BE BINDING BETWEEN KAISER MEDICAL CENTER INC., AND THE HOLDER OF THE E-CARD. BY USING THIS KAISER CARD, THE HOLDER AGREES TO ABIDE BY THE TERMS AND CONDITIONS OF THE E-CARD, AS MAY BE AMENDED FROM TIME TO TIME, AS WELL AS THE RULES AND GUIDELINES AS MAY BE IMPLEMENTED IN THE KAISER MEDICAL CENTER BRANCHES WHERE THIS KAISER CARD SHALL BE USED.
